Prior to your baby’s dedication please submit the following via email format:

 

• Baby’s Full Name

 

• Date of Birth

 

• A Picture of the Baby

 

• Parents Names

 

• Grand Parents Names

 

• God Parents Names

 

• A short Bio of the Baby
